📋 Role Overview & Responsibilities
Job descriptionActi-Kare is seeking a compassionate and detail-oriented Client Care Manager to help seniors live independently and with dignity in their own homes. As a Client Care Manager, you will ensure our clients receive the highest quality of personalized, in-home care and support.
Key Responsibilities:Serve as the empathetic point-of-contact for clients and their families, building relationships and addressing any concerns with care and sensitivityConduct thorough in-home assessments to fully understand each client's unique needs, preferences, and goals for independent livingCollaborate closely with our team of experienced caregivers to develop and regularly update personalized care plans that prioritize client comfort, safety, and quality of lifeOversee the scheduling of caregiver visits, ensuring continuity of care and monitoring for consistent, high-quality service deliveryCommunicate proactively with clients, families, and the care team to provide updates, address challenges, and coordinate care seamlesslyParticipate in the onboarding and training of new caregivers, providing mentorship and guidance to ensure they are equipped to deliver our signature brand of compassionate careThoroughly document all client interactions, care plans, and other relevant information in our electronic recordsAssist with general office duties, such as answering phones and collaborating with other care team members
Qualifications:High school diploma or equivalent, Bachelor's degree in healthcare administration, Associates Degree, or related field preferredCNA or HCA certification preferredMinimum 1 year of experience in home care, case management, or a similar client-facing roleExceptional communication and interpersonal skills, with a demonstrated ability to empathize and build trust with a diverse range of clients and stakeholdersStrong organizational skills, attention to detail, and capacity to manage multiple priorities and tasks effectivelyProficient in Microsoft Office applications and adaptable to learning new software systemsValid driver's license, great driving record, and reliable transportation to conduct in-home visitsMust be willing to drive in areas of King county, Snohomish county, and Tacoma areas to visit clients.Passion for improving the lives of seniors and a genuine commitment to Acti-Kare's mission of enabling independent living
