Schedule: PRN, require at least on average a shift a week with two shifts per month being a weekend day shift (Saturday or Sunday).
Shift: Days. Includes some weekends.

The Role You’ll Contribute
Provides patient care in their home, following physician’s orders.
The Value You’ll Bring To The Team
Initial assessment of patient in their home. Assures physician orders for care meet regulatory requirements. Ongoing evaluation of client in a holistic manner. Plans patient care in consultation with Case Coordinator Nurse. Supervision of LPNs and Home Health Aides and updates HHA Care Plans. Provide accurate, current documentation. Schedules daily visits to facilitate patient needs.
Minimum Qualifications
The expertise and experiences you’ll need to succeed:
One (1) year experience in medical surgical setting. Current nursing license in the State of Florida. Certified in Basic Life Support/Adult.
Preferred Qualifications
Home Care experience preferred.
