About Us: BlueWave Medicine is nestled in the stunning surroundings of the award-winning Cavallo Point Lodge, spanning 80 acres of national parkland beneath the iconic Golden Gate Bridge. Our practice embodies a holistic approach to healthcare, and we're seeking a full-time Care Navigator that shares our commitment to excellence.

Role and Responsibilities:As a Care Navigator, you will be at the heart of our patient-centered care model. Your role is pivotal in orchestrating and enhancing the patient journey within our high-engagement, technology-driven clinical environment. Key responsibilities include:Efficiently handling phone inquiries and appointments with a warm and caring demeanor.Coordinating patient schedules via both phone and email, optimizing their experience.Managing seamless communication with external facilities and providers, including document and referral coordination.Organizing and digitally filing electronic documents within our EMR system.Overseeing medical records, electronic documents, and relevant record sharing with referring providers.Taking initiative in ordering lab test kits and radiographic tests, actively following up on results.Maintaining up-to-date electronic health records using Elation software, including patient chart reviews for health maintenance.Preparing and organizing electronic charts prior to patient visits.Actively participating in team meetings to ensure smooth communication and timely patient follow-up.Collaborating with clinicians, medical assistants, and office managers to drive optimal patient outcomes and satisfaction.

Qualifications:High School diploma required; Associate’s or Bachelor's degree in a related field preferred.Valid California Driver’s license, reliable car, and acceptable DMV record.Strong attention to detail, commitment to patient care, and follow-through.Excellent verbal and written communication skills, fostering a positive work environment.Ability to adapt positively to change and work effectively within a self-directed setting.Experience in primary care, concierge medicine, or integrative medicine settings preferred.Proficiency in electronic medical records and standard word processing applications.Familiarity with point of service systems (e.g., MindBody software) and electronic communication tools (e.g., Slack, video conferencing).Understanding of HIPAA and healthcare-related privacy laws.
