RESEARCH NURSE PRACTITIONER / 40 HOUR DAYS / BWH-CCI

This BWH CCI Research Nurse Practitioner Position Will Work

40 hours/week (five 8 hour shifts M-F) On cutting edge clinical research, including a large focus on non-oncology cell and gene therapy studies Primarily in the outpatient setting, but will have some research participants with inpatient stays With a dedicated team of administrators, coordinators, nurses, NPs/PA, and physicians committed to providing a positive clinical research experience for research participants

General Summary/ Overview Statement

The Center for Clinical Investigation (CCI) provide research support for the BWH investigator community. The CCI is designed to provide infrastructure support to the investigative community, leading to a more effective and robust clinical research enterprise at Brigham and Women’s Hospital.

The Research Nurse Practitioner (NP) uses their research-specific clinical knowledge to offer individualized guidance to patients, families and caregivers to foster independence during their complex research protocol. Using the nursing process and the research protocol, a Research NP assesses patient needs and provides education and resources to facilitate appropriate and timely quality health and psychosocial care throughout all phases of the protocol. The Research NP creates an environment of care that is patient-centered and compassionate, inspiring and motivating.

The Research NP serves as integral members of a multidisciplinary team to provide routine and specialized research procedures and services to Harvard-affiliated research investigators.The Research NP has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care.The Research NP works under the supervision of the attending physician.

Clinical

PRINCIPAL DUTIES AND RESPONSIBILITIES

Under the supervision and direction of the responsible physician, provides research procedures to study participants in various hospital outpatient clinics and inpatient settings, as assigned and across a continuum of age and acuity.The staff member must be able to demonstrate the knowledge and skills necessary to provide care, counseling, and teaching to a select patient population served on his/her assigned unit. Performs complete and accurate health histories and physicals whenever necessary. May perform medical histories and physical exams as a member of the study team for an IRB approved research protocol. Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients’ health care needs. Orders medications and writes prescriptions according to research protocols. Manages care and treatment of study participants as indicated in the professional scope of practice and within current standards of medical practice protocols. Maintains accurate documentation of patient history, physical exams, treatment, and progress in medical records. Exercises autonomy in decision-making by using sophisticated clinical knowledge to demonstrate a high level of accountability to supervising physician, research investigators, other staff, and research participant. Appropriately responds to emergency situations within the scope of practice and implements urgent staff consultation and/or activation of BWH code system. Collaborates with multiple disciplines in the development and implementation of protocol procedures to provide optimal patient care.Performs diagnostic, therapeutic, or other procedures as outlined in research protocol. Provides follow-up to research investigators/acts as liaison between the research center staff and the study investigators to address patient problems and responses to treatment. Facilitates communication between various health team services and the study participant. Delivers expert clinical care through evidenced-based practice partnerships with research participants, families and colleagues.

Research Navigation and Research Participant Education

Participates in day-to-day operations of the program including patient education and call triage, focused on the research activities of protocols affiliated with the CCI. Assumes navigational responsibilities of the research participants in protocols affiliated with the CCI for (but not limited to) assessments, education, coordination, and documentation. Facilitates individualized care within the context of functional status, cultural consideration, health literacy, psychosocial, reproductive/fertility, and spiritual needs for patients, families and caregivers. Supports a smooth transition of research participants from active treatment into follow up care, survivorship, management, or end-of-life care. Assists research participants with issues related to treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical framework that is nonjudgmental and nondiscriminatory. Acts as a liaison between the research participants, families, and caregivers and the providers to optimize outcomes. Advocates for research participants to promote patient centered care that includes shared decision making and patient goals of care with optimal outcomes Provides psychosocial support to and facilitates appropriate referrals for research participants, families and caregivers Effectively communicate routine to complex study information to research participants. Act as liaison between research participants and other members of the team regarding patient care activities relating to the research protocol Provide consultation to primary physicians, nurses and technicians regarding protocol implementation. Work with physicians to determine action plans. Draft source documentation worksheets as studies require. Work with Trial Pharmacist and others to develop templates for studies that require templates. Functions as the primary nursing contact for care delivered within the physician practice setting. Engages in collaborative decision making, utilizing available data expertise and wisdom of the collective Manages patient care across the continuum, including new patient calls, and making appropriate referrals as necessary. Works collaboratively and functions as an effective member of the health care team. Monitors the environment of care with attention to patient safety and assures compliance with regulatory agency standards. Maintains clinical practice that is evidence-based and consistent with nursing policies, procedures and standards. Provides care based on age-specific needs of the patient. Complies with accepted standards of professional nursing practice that is evidence- based and consistent with policy, and procedures determined by BWH as well as with all licensure/registration requirements applicable to the nursing role. Fosters an environment that is sensitive to the needs of diverse populations, including but not limited to culture, ethnicity, gender, and age.

Quality Improvement

Participates in the QA/QI projects for the CCI. Participates in CCI evaluation and development of strategic goals consistent with BWH mission and professional values. Participates in the tracking and monitoring of metrics and outcomes, in collaboration with administration, to document and evaluate outcomes of the navigation program Guides the direction of continuous quality improvement and research initiatives. Establishes and monitors care standards, evaluates outcomes, makes recommendations for changes, and works to implement changes under the direction of the supervisor. Collects, documents, and analyzes study participant data as necessary for QI monitoring. Seeks and provides consultative services as required. Advocates for quality of care.

Other Responsibilities

In conjunction with other CCI staff, design and/or implement new information systems that improve the efficiency of operations provide easier access to data, eliminate redundancy, etc. Provide substantial administrative support to prospective investigators in preparing their applications for use of the CCI and determining how the CCI can be utilized for their study. Represent the Brigham and Women’s Hospital at any necessary local, regional, and national Meetings. Maintain a current knowledge of Federal (HIPAA, GCP, FDA, etc), State, and Hospital laws, regulations, and guidelines governing human subject research; attends educational and professional seminars. Assure compliance with BWH, JCAHO, NCRR, OSHA, DPH, and state and federal regulations. Undertake special projects assigned by the CCI Executive Director, Vice President of Clinical Research, the Senior Vice President of Research Compliance and Innovation and the Chief Academic Officer. Assists in orienting new staff and evaluating clinical competence of staff. Provides leadership and coaching as appropriate. Practices professional behavior at all times. Appropriately interacts and communicates with all members of the healthcare team, patients, and families. Takes direction well and understands own limitations. Demonstrates good ethical judgment. Performs other related duties as assigned or directed to meet the goals and objectives of the organization.

Education

Must have successfully completed a Nurse Practitioner program required. Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioners. Master of Science of Nursing (MSN) required. Adult-Gerontology NP or Family NP (AGNP or FNP) required.

Experience

Prior Research Experience as a Nurse Practitioner required. Hematology/Oncology experience as RN or NP required. Infusion Experience as RN or NP required. Ambulatory experience as RN or NP preferred. 2-4 years of nursing experience as RN or NP preferred. History and Physical experience preferred. Cell or gene therapy experience preferred. Experience providing consultation to primary physicians, nurses and technicians regarding protocol implementation preferred.

Other

ANA or other appropriate nursing certification in area of practice and authorization by the Massachusetts Board of Registration in Nursing to practice in the expanded role. DEA for prescription writing. Professional nursing certification preferred (OCN, BMTCN, CPHON, AOCNP, CRNI, RN-BC) AHA BLS required (AHA ACLS accepted as minimum requirement)

Each NP shall demonstrate the following professional knowledge and skills:

Interpersonal skills needed for interacting effectively with physicians, staff, patients, and their family. Analytical skills needed for evaluating study participants and coordinating study procedures in accordance with each protocol. Is a strong team player and supports other providers and offers help to the group. Proficiency in BWH computer systems as needed to independently perform daily tasks such as order entry, Epic, lab tests and results, incident reporting, etc. Ability to consistently provide high quality and timely documentation, including admission notes, progress notes, discharge summaries, and dictation.

NP should demonstrate the following physical and mental abilities required

Must be energetic, quality driven and have proven ability to be a productive member of a cohesive team of peers, dedicated to providing optimum patient care. Understanding of procedures, policies and documentation required to ensure compliance with hospital standards of care. Ability to accept instructions and guidance and assume graded responsibility in a complex environment. Professional attitude towards patients and co-workers at all times and a collegial working relationship with other personnel involved in patient care.

Each NP shall demonstrate the following clinical knowledge, skills and competencies:

Must exercise the highest degree of professional judgment under the direct supervision and guidance of a licensed physician. For the most common diagnoses admitted to research, demonstrate competency to effectively diagnose, formulate appropriate treatment plan, and efficiently implement the plan without much direction, and with a greater level of independence, from the attending physician. Broad knowledge of medical environment, standard research procedures, and performance goals. Ability to administer high quality care and treatment and exercise such independent originality in solving problems not covered by existing guidelines, within legal limits, to meet the needs of the patients. Working Conditions

Work areas will include the Center for Clinical Investigation discrete inpatient and outpatient units, BWH hospital clinics, and various inpatient units, including but not limited to the Neuro ICU, Surgical ICU, and the Cancer Center.

Job responsibilities will require significant amounts of standing, moving between rooms and floors, and communicating verbally and in writing with colleagues, patients, and family members. Patient care responsibilities will require standard precautions, and in some cases additional precautions as necessary.
