Member Connections Representative I (San Antonio, TX)

This is a remote/work-from-home position with some local travel required in the Bexar County (San Antonio, TX) region up to 10%.

Position Purpose Assist the health plan in establishing a presence in the community through home visits and community outreach.

Educate members on health care benefits through home visits and outreachAssist members with activities related to accessing careDocument information and activities in applicable systemsDeliver Connections Plus® cell phones, teach and answer questions related to cell phone use and navigationCoordinate and implement community events, including Start Smart Baby showers and Birthday Parties, school events, healthy life style events and various community initiativesSchedule and complete home visits for noncompliance, benefits information, ED and readmission diversion and other high risk issuesConduct targeted phone calls for compliance, HEDIS initiatives and health risk assessmentsFollow-up with appropriate parties on status of high risk member referralsAssist in finding and connecting members to community social services and agenciesAbility to travel throughout designated region

Education/Experience High school diploma or equivalent experience. Bilingual ability preferred. 2+ years of social services, community outreach, or education experience.

License/Certification Valid driver's license and automobile insurance. Some locations may require Community Health Worker certification, CPR certification, and/or bi-lingual.

Pay Range $18.66 - $31.73 per hour
