Job Description

The PACE Home Care RN under the direct supervision of the Director of Clinical Operations, is responsible for the development and implementation of home-based services for program participants including the provision and maintenance of durable medical equipment and the supervision of nursing home care. The PACE Home Care RN collaborates with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care. The Home Care RN works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe patient environment, and contributes to a healthy work environment.

Responsibilities

Coordinates and supervises nursing care provided to participants who are in contract nursing facilities according to CMS and/or Virginia Standards. This level of coordination includes interaction with employees of contracted nursing facilities

Assesses the home care needs of frail elderly participants and develops specific plans of care to address identified needs

Works with the Home Care Scheduler to coordinates the implementation of home care services, including personal care, nursing homemaker-chore services, occupational therapy, physical therapy, and speech therapy to make sure quality services are provided according to the needs of individual participants

Works with Home Care RN Supervisor to facilitate monitoring and supervising all home care CNA's at the respected PACE site

Serves resource to all home care nursing staff by performing joint home visits to promote development of staff

Authorizes, coordinates, and oversees maintenance of durable medical equipment used by participants

Conducts initial and periodic assessments of participant needs

Participates in the development of the participant plans of care, the coordination of a -hour care delivery system, and the IDT Plan of Care

Communicates participant changes to team members

Completes all documentation in the medical record as required

Investigates complaints and incidents in a timely and positive manner and resolves issues, as able

Meet a standardized set of competencies for the specific position established by Centra PACE and approved by CMS before working independently

May perform other duties as assigned or requested and job specification can be modified or updated at any time

Qualifications

Required Education: Graduate of an accredited Nursing Program

Preferred Education: Bachelors of Science Degree in Nursing (BSN)

Required Experience: Two (2) to five (5) years of nursing experience with at least two (2) years of supervisory experience and one (1) year of caring for the frail or elderly. The Home Care Coordinator must be able to function smoothly on a team, possess good organizational communication and supervisory skills, and effectively handle difficult and unusual interpersonal situations

Preferred Experience: Two (2) years of the nursing experience to be in home health

Required Certifications and Licensures: Current Virginia or Multistate Nursing Registration. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.

Effective April 1, 2024, candidates seeking employment in this role at Centra must possess a valid and current American Heart Association Basic Life Support (AHA BLS Provider or AHA BLS Instructor) course completion card. Proof of certification will be mandatory prior to commencing employment. Both AHA and RQI cards are acceptable forms of certification.
