Summary
A 52-week post-graduate educational training program under the direction of an advanced pharmacy preceptor(s).
Responsibilities
Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders. Develops knowledge, skills, and experience to become an advanced pharmacy clinician who manages and improves the medication use process ensuring safe, appropriate, and cost-effective therapies for patients. Develops and expands upon the knowledge and skills required to provide evidence based, patient-centered medication therapy management within interdisciplinary teams. Develops leadership skill to effect change within the pharmacy clinical and operation sections of acute care hospitals. Expands upon and improves the pharmacotherapeutic knowledge and communication skills required to provide medication, therapeutics, and practice related education. Develops the knowledge and skills to utilize medical informatics by utilizing the informatics tool available at CHRISTUS. Assumes roles and responsibilities also listed in Clinical Pharmacist I job description.
Requirements
Graduate from an ACPE accredited Doctor of Pharmacy Program required Experience Must be licensed or eligible for pharmacist licensure within 90 days from first day of employment unless a formal program extension is approved for extenuating circumstances, as guided per ASHP. BLS and either ACLS or PALS (if pediatric practice) within 180 days of start date.
