Independent Living Skills Coordinator

Position Overview: As an Independent Living Skills (ILS) Coordinator with Clear Skies Solutions, you will play a pivotal role in empowering our clients to lead independent, fulfilling lives. Through personalized assessments, you'll connect individuals with the resources and support they need to achieve their independence goals. Your patience, compassion, and resourcefulness will guide our clients towards a future filled with dignity and opportunity.

Key Responsibilities:Conduct comprehensive assessments to identify client needs and strengths, creating tailored service plans.Build cooperative relationships with clients, their families, and external service providers to ensure successful plan implementation.Connect clients with essential community resources for housing, employment, healthcare, and transportation.Provide continuous support through regular check-ins, reassessment of goals, and effective crisis management.Maintain accurate and timely records of client interactions, services provided, and progress reports.Develop strong relationships with community partners to expand our network of support.Participate in professional development activities to enhance your skills and knowledge.

Qualifications:Bachelor's degree or 1 year of relevant experience, especially with adults with developmental disabilities.Outstanding communication and interpersonal skills, with the ability to form strong relationships with a diverse client base and their families.Ability to work both independently and as part of a team, demonstrating a commitment to collaborative problem-solving.Strong organizational and analytical skills, capable of juggling multiple priorities effectively.Experience with diverse populations and an understanding of the unique challenges faced by adults with developmental disabilities.A valid driver's license and reliable transportation for community outreach.
