Family Nurse Practitioner

Coras was Founded in 2021, as a limited liability corporation as a subsidiary of Conexio Care, Inc., CORAS Wellness & Behavioral Health provides a wide array of substance abuse and behavioral health treatment services, including but not limited to, MAT Services, Outpatient Services, Residentials Services, Sober Living Services, Primary Care Services, and other wellness and behavioral health services. We currently have 16 facilities throughout the state of Delaware.

POSITION STATEMENT: The Family Nurse Practitioner will advance accessible, person-centered, equitable, high-quality health care for diverse communities through practice, education, advocacy, research and leadership. RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS: Direct Duties: The Family Nurse Practitioner will be responsible for the following:Complete and document Comprehensive Physical Exam on all new clientsComplete and document Yearly physical exams for current clients; to include lab workReview yearly labs and recommend treatments if needed; new labs ordered and reviewed as neededDiagnose and Treat clients who present with medical concernsPrescribe medication as needed for current clientsPerforms other duties as discussed and agreed upon in writingOther Responsibilities:Communicates clearly and effectively to patients and their family members, guests and other members of the health care team;Exhibits therapeutic rapport with patients maintain professional boundaries, being respectful of personal space, giving patients direct attention while maintaining rational detachment;Encourages mutual trust, respect and cooperation among team members;Displays a competent knowledge of Agency policy and procedures as well as Federal and State Regulations. Keeps current with available resources.Encourages staff/clients while demonstrating self-initiative in problem solving.Maintains confidentiality, proper boundaries and an ethical relationship with clients and co-workers according to Company policies and their Delaware Professional Board regulations.Ensures all client records are maintained according to agency standards, policies and procedures.Engages in professional development. Responsible for keeping current in the field. Completes seminars and/or continuing education programs, as appropriate to update knowledge about addiction and mental health services.Ensures client’s file documentation and other reports are up to date.Completes annual educational program as required.Participates in the development of quality improvement activities designed to enhance agency and program servicesEngages in professional development. Responsible for keeping current in the field by attending formal and agency trainings.Performs other duties as discussed and agreed upon in writing

MINIMUM EDUCATION AND EXPERIENCE QUALIFICATION REQUIREMENTS:Graduate of Family Nurse Practitioner program from an accredited college or universityCurrent professional license per state regulationsDEA and/or applicable waivers required for prescribersPreferred experience in behavioral health or substance abuse treatmentNo history of alcohol or drug misuse for a period of three years prior to employmentFive years of clinical/administrative experience in providing outpatient services and behavioral health servicesThree years of experience providing clinical supervisionValid Delaware clinical behavioral health professional license or CADC with supervision experienceClinical Supervision credential preferred (not required)Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractionsCertification in First Aid and CPRExperience with underserved populations preferredExcellent communication skills and organizational skillsExcellent ability to work with clients with substance abuse, co-occurring and/or mental health disordersExcellent writing skills and record keeping skills
