Contract/Travel Physical Therapist
Location: Wisconsin
Setting: Mixed - Inpatient Acute, Swing Bed, Outpatient, Skilled Nursing Facility (SNF) (Weighted more heavily towards outpatient; Current staff willing to adjust load as needed/desired)
Start Date: ASAP
Schedule: Monday to Friday, 40 hours guaranteed. Flexible hours, mainly 8:00 AM - 4:30 PM, but will accommodate adjustments.
Case Load: 8 patients per day
Culture: Small facility in a rural setting, fostering a supportive and collaborative environment. No strict productivity standards. Physical Therapists work closely with in-house providers.
Job Description
Our facility in Wisconsin is seeking a dedicated and experienced Contract/Travel Physical Therapist to join our team. As a Physical Therapist in our mixed setting, you will have the opportunity to work in various environments including inpatient acute care, swing bed, outpatient, and skilled nursing facility (SNF), with a focus on outpatient care. We prioritize patient care and believe in giving our therapists the flexibility to provide individualized treatment plans without strict productivity constraints. Responsibilities
Evaluate, plan, and administer comprehensive physical therapy modalities/regimens to patients in a variety of settings. Develop and implement individualized treatment plans based on patient needs and goals. Collaborate closely with in-house providers to ensure coordinated and effective patient care. Maintain accurate and up-to-date patient records.
Requirements
Valid Physical Therapist license in the state of Wisconsin. Experience in a mixed setting, particularly outpatient care, is preferred. Strong clinical and interpersonal skills. Ability to work independently and as part of a collaborative team. Excellent communication and documentation abilities.
