Job Description
Provide compassionate and holistic nursing care to patients receiving hospice services, focusing on pain management, symptom control, and end-of-life care. Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans in collaboration with the interdisciplinary team. Administer medications, perform skilled nursing procedures, and provide education and support to patients and their families to promote comfort and dignity throughout the hospice journey. Coordinate care with physicians, nurse practitioners, social workers, spiritual counselors, and other healthcare professionals to ensure coordinated and effective care delivery. Document patient assessments, interventions, and outcomes accurately and timely in accordance with regulatory standards and agency policies. Participate in team meetings, case conferences, and in-services to enhance clinical knowledge, promote interdisciplinary collaboration, and improve patient care quality.
Requirements
Graduate of an accredited nursing program with a valid Registered Nurse (RN) license in the state of Colorado. Minimum of one year of nursing experience in hospice, home health, palliative care, or a related field preferred. Current CPR certification; additional certifications such as Certified Hospice and Palliative Nurse (CHPN) are advantageous. Strong assessment, communication, and interpersonal skills with a compassionate and empathetic approach to patient care. Ability to work independently and collaboratively within a multidisciplinary team environment. Proficient computer skills and familiarity with electronic medical records (EMR) systems.
