Position Details:Eisner Health is seeking an experienced Family Medicine, Adult, or Internal Medicine Physician who will provide services to the underserved communities of the San Fernando Valley. This distinct position will provide multi-generational care in an environment that promotes excellent support services including dental care, mental health, and social work support. A successful candidate will be responsible for providing the full range of comprehensive outpatient and family practice medical care including direct patient care, multidisciplinary interaction with support services, and other clinical and administrative responsibilities.

Job Duties:The physician as the Primary Care Provider (PCP) who has been assigned to the patient, generally on the basis of patient choice or rotation, shall see the patient at the earliest opportunity. Their responsibilities are to:Take care of the patient’s immediate medical needs.Undertake medical evaluation, including history and physical examination, laboratory, X-ray, referrals, or other diagnostic studies.Evaluate the patient and develop a problem list.Develop treatment objectives and plans.Initiate treatment orders and procedures.Educate and counsel patients to promote positive health behaviors and disease prevention.Work closely with clinical staff in coordinating and obtaining needed services for patients and families.Consult with the Director of Adult Medicine as necessary.Perform additional duties as assigned.

Requirements and Qualifications:Must have a current license to practice medicine in the State of California.Board Certified/Board Eligible in Internal Medicine or Family Practice.Must have a current DEA License.Proficient in English and medical Spanish preferred.Must possess a current CPR Certification.Effective communication and interpersonal skills with patients and families.Experience in working with Electronic Medical Records, preferred.
