Job SummaryThe Practice Transformation Engagement Specialist supports the work of the Practice Transformation team at Elevation Health Partners. The Practice Transformation team works to improve patient care by implementing prescribed evidence-based care management and practice transformation strategies among Federally Qualified Health Centers (FQHCs), physicians and other patient care providers in meeting quality performance objectives through data collection, data analysis, process outcome evaluation, and other PDSA rapid cycle quality improvement methods. The Practice Transformation Engagement Specialist works to carry out the goals of the project, by working collaboratively with practices, other consultants, health plan divisions and departments, government, vendors, and community collaborators.

Essential Functions Support the Practice Transformation Team by coordinating meetings, schedules, activities, communications, and other administrative tasks as neededConducts outreach campaigns to practices when launching new services or Medi-Cal based technical assistance and grant programsImplement strategies such as quality improvement interventions, empanelment, care teams, QI dashboards, workforce strengthening, care coordination and other practice-level changesAssist and monitor assigned practices in the implementation of QI initiatives and processes PDSA cyclesImprove performance on HEDIS measures and P4P programsDevelop care coordination protocols, including social linkagesWork directly with lead physicians and their staff to address issues related to quality improvement and care processes within their practicesResearch and educate practices about models of best practice and replicate model elements as needed in the practicesImplement measures and develop tools to gauge a practice’s ability to carry out QI processes, such as disease management process flow charts and protocols for case management and care coordinationGather appropriate data from QI reports, audits, and/or outside sources and share them with physicians and their practice staffActively participate in project steering committee meetingsPerform required medical chart audits and enter results into appropriate databases to facilitate retrieval and analysis by local projectAssist in the implementation of QI initiatives and processes at the practice levelWork directly with providers to assist them in the development of disease management initiatives and protocolsProvide staff training as needed to implement QI processes and initiatives

Classification, Benefits and CompensationThis is a full-time, exempt position and will report to a Principal/Director. Candidates must be authorized to work for any US employer. The Practice Transformation Engagement Specialist must have residency in a U.S. state with a Pacific Standard Time (PST) time zone. Elevation Health Partners supports a work-from-home arrangement, subject to client and/or project needs.

Elevation Health Partners is committed to paying competitive wages. The Practice Transformation Engagement Specialist role corresponds to a level with an annual salary of $55,000-$70,000, depending on the candidate's education and (Bachelors or Masters) years of experience.
