Program Consultant Supervisor

Description

Perinatal Health Partnership Program is a home visiting program for high risk pregnant and postpartum patients and their infants. The goal of perinatal home visiting services are to improve maternal and infant outcomes through clinical assessment and monitoring, education, linkage to needed services and resources, and timely identification and referral for warning signs and symptoms. This program will provide home visiting services for up to one-year post-partum.

This is a full time, full benefit eligible position, located in Lowndes County, Georgia. This position is Grant Funded and will be considered for renewal on a year to year basis.

Job Responsibilities

Supervises, guides, and/or instructs the work assignments of subordinate staff. Provides technical assistance and training within a public health nursing care specialty. Plans, implements and supervises a specialty nursing program. Evaluates programs for compliance with state and/or federal guidelines.

Additional Duties

Serves as lead for the pilot program. Serves as liaison to the state office, responsible for reporting requirements such as annual work plan and quarterly reports. Supervises employees. Ensure all program requirements are met.

Minimum Qualifications

Master's degree in nursing, public health or related field from an accredited college or university AND Four years of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification OR Bachelor's degree in nursing from an accredited college or university AND Six years of nursing experience AND current Georgia Professional Nurse license AND CPR certified OR Four years of experience at the lower level PH Nursing Prog Consultant 3 (HCP212) or position equivalent AND current Georgia Registered Professional Nurse license AND CPR certified.
