Join our team at a growing Home Health Care Organization nestled in the heart of Michigan! This thriving patient-focused facility is seeking a dedicated Direct Home Care Worker to enhance our patient care team.
Responsibilities
Under supervision, provides supportive services to participants in various settings. Follows individual Care Plans for each participant. Assists in maintaining physical and emotional comfort and promoting independence. Performs a range of tasks including personal care, meal assistance, medication reminders, and transportation support. Ensures accurate and timely documentation of care provided. Responds to emergencies and reports deviations promptly.
Qualifications
High School Diploma or G.E.D. required, with a minimum of 2 years relevant experience or completion of a formal training program. Participates in annual in-service training and screenings. Demonstrates interpersonal skills and respect for participants and colleagues. Flexible scheduling ability and sensitivity to diverse backgrounds. Must meet standardized competencies and maintain medical clearance for direct participant contact.
Working Conditions
Requires driving within designated area with reliable transportation and valid driver's license. Variable working conditions with potential exposure to communicable diseases and chemicals.
