Role DescriptionThis is a full-time on-site role for a Medical Case Manager at Harmony Healthcare Inc. located in Orlando, FL. The Medical Case Manager works closely with a diverse patient demographic, including those living with HIV, Hepatitis C, multiple psychosocial and/or health related needs. Utilizing a strength based approach, this role is responsible for the identification, assessment, monitoring, and intervention of patient needed treatments, support and resources. A primary goal is to lead patients to overall improved health through our program using coordination of timely access to comprehensive medical care and to social services with a fully supportive engagement for patient compliance with provider instruction, appointment keeping and resource eligibility requirements. In this role, the MCM will be an integral part to the patient's treatment plan at our clinic.

Essential Functions

Works in collaboration with the Program Manager and Clinical Staff to identify current and new clients who can benefit from Case Management services. Face-to-Face Patient Case Management: Intake Screening verbal/written, Orientation, Assessments, Regular Updates to Action Plan; continually review case progress, facilitating next steps and determine case closure when necessaryFacilitate multiple care aspects (case coordination, information sharing, referral to outside resources, risk, prevention treatment and disease education, etc.)Help patients make informed decisions by acting as their advocate regarding their care and treatment optionsDevelop effective working relations and cooperate with medical team throughout both: the entire case management and clinical pathwaysTake the extra effort and go the extra mile building a professional relationship, trust and inspire patient progress.Record case information, accurately, complete all necessary forms and produce statistical reportsPromote quality and cost-effective interventions and outcomesAssess and address motivational issuesIdentify and develop relationships with external organizations and providers increasing our patient resources network.Adhere to professional standards as outlined by protocols, rules and regulations including participation in all required training and certifications including, but not limited to: HIPAA, Biohazardous Waste, CPR/BLS, 500/501 Rapid HIV testing, and Rapid HCV testing.

Major Functions/Accountabilities

Assistance accessing health insuranceEducation, testing and counseling about HIV and Hepatitis C transmission, chronic medical, risk reduction, and harm reduction education.Assistance with accessing support services: housing assistance, transportation, etcMedical case management of HIV medication therapy to include client education concerning risks and side effects, monitoring client adherence, and tolerance of medicationsAdherence assessment and interventions to include counseling, education, and referral as appropriate, including working with in-network pharmacyEducate on safer sex practices, clean needles to prevent transmission of sexually transmitted diseases and Hepatitis CParticipate in Community Outreach events such as College Health Fairs, GayDays, Pride Events, etc.Attend and participate in HIV Community Agency and Care Collaborative Meetings, as scheduled. Skills

Bilingual preferred (English and Spanish and/or Haitian Creole)Proven working experience in case management, or a related jobProject Management experience, education and/or skills.Excellent knowledge of case management principles, healthcare management and business savvyConfident with working with clients with a substance abuse history, and/ or HIV riskAbility to positively work effectively with othersEffective communication skillsExcellent organizational and time management skillsProblem solving skills and ability to multi-task

Qualifications

Knowledge of the service needs of individuals affected by HIV/STI/AIDS, PrEP or Hep C who may or may not have a history of substance abusePassion and commitment to help those at risk and in need of servicesAbility to thrive in a fast paced environmentAbility to adhere to a zero tolerance drug use policy, and successfully pass random drug screening.Comfortable working in different and varied work environments, such as a clinical settings and MAT centersAt least one (1) to two (2) years of experience in working with organizations that serve individuals affected by hepatitis, HIV or STIKnowledge of PrEP, HIV care, Hepatitis C care, opiate and other addiction recovery for substance abuse or alcoholLicensed Practical Nurse (LPN) with at least one (1) year of experience preferredA master’s/bachelor’s degree in non human services field and one (1) to two (2) years case management experienceA master’s/bachelor’s degree in a human services fieldValid Driver's licenseProficiency with Microsoft Office products (Word, Excel, Powerpoint, Teams, etc.)
