Job Summary
The Care Coordinator is responsible for assisting providers and staff in providing superior care to homebound clients. Care Coordinators can support individual clinical teams or specialize in a specific process such as durable medical equipment coordination.
Essential Responsibilities
Patient Communication
Receives in-coming calls for clinicians; documents and follows clinicians’ direction regarding communication with patients, families, and vendors. Contacts patients, families, and caregivers per clinician specific instruction. Outreach may include coordination of care, test results, referral information, appointment rescheduling, and documented instructions from the clinician. Links patient, families, and caregivers with other community resources including HCP social worker to enable continued community- based care and to enhance their quality of life.
Clinical Support
Assists clinicians with referrals to specialists and orders, including but not limited to home health, hospice, palliative care, diagnostic testing, medical supplies, and equipment. Assists Housecall Providers staff with patients on their panel, provides support to clinical team working with assigned providers. Works collaboratively with the transitions, palliative care, and hospice teams to ensure excellent communication and continuity of care. Assists clinicians with obtaining prior authorizations. May authorize prescription refills for non-controlled drugs if approved by clinicians. Assists providers and clinicians with arranging patient transportation and scheduling of interpreters. Communicates with transitions or palliative care teams regarding patient hospitalizations and clinician visit requests, or equipment needs. Sends chart notes, specialist referrals, visit summaries and forms for clinician signatures as requested. Assists with complaints that might come in from patients, families, or caregivers; documents as directed and forwards to appropriate staff member.
For Those With a DME Care Coordinator Focus
Responsible for meeting with vendor reps to maintain vendor relationships and assist with problem-solving, for regularly reviewing CMS documentation guidelines and then educating the CC team/clinicians about changes, and for tracking changes to the Medicare competitive bid. Able to request DME equipment, monitor and request clinician charting to expedite getting equipment ordered, follows up to make sure equipment is delivered.
Documentation and Other Duties
Documents all patient interaction and support activities in an appropriate and timely fashion to maintain patient’s medical record. Maintains patient/family confidentiality in compliance with HIPAA laws. Learns and provides back-up coverage for reception, DME, and office assistant responsibilities.

Experience and/or Education
Minimum 3 years’ experience in a medical field, geriatrics, home health, or hospice setting, including experience with medical records system
Knowledge, Skills And Abilities Required
Knowledge
Knowledge of medical terminology, basic patient care, and community resources Understanding of electronic medical record systems and the ability to integrate updates to those workflows on a regular basis Knowledge of HIPAA privacy rules and regulations
