Job Description: Nurse Practitioner, Primary Care

The Nurse Practitioner, specializing in primary care, will work closely with the Medical Director (MD) to provide excellent clinical services to various IntraCare Health Center locations. They will direct, implement, and provide a plan of care for patients at clinics. They will provide general preventative care, conduct check-ups, treat both acute and subacute illnesses, order lab tests, and prescribe medication for patients.

Key Responsibilities

Performs physical exams and patient observationsRecords patients' medical histories and symptomsCreating patient care plans and contributing to existing onesOrdering, administering, and analyzing diagnostic testsMonitoring and operating medical equipmentDiagnosing health issues and plan of care for patientsAdministering medicine and other treatmentsDetecting changes in patients' health and modifying treatment plans, as neededConsulting with healthcare professionals and familiesTraining patients and their families to manage and prevent illness and injuryPrescribes appropriate pharmacologic and non-pharmacologic treatment modalitiesFacilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patientsFacilitates staff, patient and family decision making by providing educational toolsMonitor and provider start-up status of clinicsProvide coverage to additional clinic locations, as needed and assignedAll other duties as assigned

Education & Experience

A minimum of 2 plus years’ experience in family medicine as NP/PA Graduate of an accredited school of nursingGraduate of an accredited Nurse Practitioner ProgramCurrent Registered Nurse Licensure in State of TexasCurrent APRN/Nurse Practitioner Licensure in State of TexasCurrent Board Certification in adult primary care or family practice specialty areaCurrent Medicare provider enrollment with active PTAN numberCurrent TX Medicaid provider enrollment with active TPI numberCurrent provider card in Basic Life Support (BLS)Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hireCurrent DEA License, or ability to obtain DEA license upon hireExperience in geriatric care preferred Previous experience with Athena EMR, preferredStrong computer skills, including Word, Excel, and PowerPoint
