Job Summary

The Critical Care Pharmacist works under the supervision of the Director of Pharmacy. He/She assists the Director of Pharmacy with achieving department goals and objectives regarding appropriate use of agents utilized to treat patients in the critical care setting. The Clinical Specialist Pharmacist provides collaborative pharmaceutical care to hospital patients through collaboration with medical staff, nurses, Pharmacy Services, ancillary service providers and Hospital Administration.

Minimum Qualifications

Minimum Education: PharmD Degree from a US accredited college of pharmacy.

Required Certifications/Licensure: Licensed as a pharmacist in the state of South Carolina; ACLS certified or become ACLS certified after hire for critical care clinical pharmacy specialist.

Required Training: Completion of Pharmacy Practice Residency or equivalent experience in their practice setting; Maintains competency in pharmacotherapy in their practice setting.

Essential Functions

Provides consultation to hospital committees, such as Pharmacy and Therapeutics, to provide a coordinated approach to drug policy management related to medications utilized in the hospital. Medication formulary management for their practice setting. Policies, guidelines, protocols and order sets relating to patients serviced in their specific area Collaborate with physicians and Pharmacy Services to ensure Lexington Medical Center (LMC) is consistent with Best Practices and other regulatory and government agency requirements related to medications used to treat patients requiring clinical pharmacy services in their practice setting. Implementation of “Best Practices” for use of these agents Implementation and education regarding safe and effective use of these agents Implementation of “Evidence-based Practices” for appropriate use of these agentsDevelop relationships with physicians and use those relationships to ensure patients receive appropriate pharmaceutical care. Develop relationships with ancillary service providers (Nutrition Services, Respiratory Therapy, Dialysis, etc.). Utilize these relationships to develop appropriate treatment guidelines and care of patients.

Duties & Responsibilities

Assist medical staff to optimize patient care with cost-effective therapy in patients receiving medications specific to their area of practice. Maximize patient outcomes and minimize unintended consequences of medication therapy. Collaborates with Center of Best Practice, nursing, medical staff and hospital administration to prepare their specific area(s) for DNV accreditation surveys and respond to any deficiencies identified. Ensure Hazardous Waste or Hazardous Waste Pharmaceuticals are discarded, stored, and managed with other compatible waste streams in containers that are properly labeled and closed prior to being shipped offsite for proper disposal. Initial RCRA training is received within 6 months of the hire date and continuing with annual RCRA training. Performs all other duties as assigned.
