Position Summary

The Behavioral Health Associate works in conjunction with the treatment team in developing, coordinating, and disseminating continuing care plans with patients. Maintains up-to-date referral network.

Primary Responsibilities

Meets with patients to research discharge planning needs as presented by the treatment team. Meets one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management. May be assigned to conduct educational lecture or group activity. Communicates with Business Development Department and referring provider to coordinate appropriate aftercare resources. Under the guidance of the Clinical Director, reviews and researches aftercare resources. Supports treatment team and patients in communication with referral and continuing care resources. Documents continuing care plans, communications and meetings. Prepares and distributes copies of patient schedules weekly as requested. Attends treatment team meetings as assigned.

Education, Skills and Experience Requirements

Bachelor’s Degree in Behavioral Science or related field preferred. One year's experience working in a behavioral health care facility preferred. Ability to provide excellent service to internal and external customers. Ability to understand and comply with government, regulatory, and company rules. Ability to maintain confidentiality of information, multi-task, coordinate projects and record/transcribe general and specific meeting minutes. Occasionally needed to work irregular hours or additional hours.
