Bilingual Remote Nurse Case Manager (RN) - 203595

We are looking for a self-motivated and passionate RN for our Full-Time Care Manager opening. This is an exciting and dynamic position, as the Care Manager, you will be responsible for ongoing assessment of health, environmental, and psychosocial status of members assigned to you, and ongoing care management of members through telephonic contact.

OVERVIEW:The CM Nurse is responsible for all forms of care management, such as care planning, coordination, and assessment of health for a managed long term care plan in NY. The Nurse will work fully remote. To be considered, the candidate requires Health plan MLTC experience and is Bilingual speaking.

ESSENTIAL DUTIES:All forms of care management, such as- care planning, coordination, and assessment of health. You will monitor services for quality, effectiveness, and progress toward goals.Coordination of transitions for patients including discharge planning from the hospital or nursing home.Identifies health, environmental, and psychosocial risks for assigned members and recommends appropriate interventions to the Interdisciplinary Care team.

JOB SPECIFICS:Requirements: NYS RN License and one year job related experience in Managed Care, Home CareMust have Managed Care MLTC Health Plan experienceHours: Monday - Friday 9:00 AM - 5:00 PM (No part time opportunities are available at this time)Location: Fully Remote (plan based out of NY)Salary: $95,000Direct Hire: Includes full benefits (medical, PTO, retirement plans)Equipment provided!

QUALIFICATIONSRN Active License in NYMust be Bilingual speaking (Spanish, Bengali, or Chinese)Must have 1 years of Healthplan Case Management experienceFamiliar with Medicaid/MedicarePassionate attitude, communication skills and problem solving will be vital to this position.
