Please feel free to schedule a first phone screen to go over details and see if this is the next step for you!I will ask for an updated resume once a call is scheduled. snshqsc.co/Eb4xAtIdeal candidates have -3-5 years experience working with older adults. -Bilingual in Spanish -CA Drivers License & Personal transportation -CRM/EHR experience-Organized with great clerical skills -Experience delivering coordinated care for older adults Expectations for this RoleCollaborate with Outreach Specialist and IDT to ensure successful enrollment Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care Conduct home safety evaluations of the prospective participants’ home Participate in Enrollment Eligibility meetings to review Level of Care recommendations Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCSMaintain accurate documentation and record all evaluations in CRM software Participate in all PACE Audits by DHCS and CMS Support the Center Director to develop participant retention strategies Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program Support business development and census growth by participating and assisting with outreach programs as needed Other duties as assigned Must Have Skills/QualificationsPassion for delivering amazing coordinated care to frail elders and disabled older adults Associates degree from accredited nursing school; BSN preferred Active Registered Nurses License in the state of California Current CPR certification or able to obtain certification within 30 days of hire 3-5 years of healthcare experience and experience working with frail elderly Require a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law. Excellent organizational, communications, written and verbal communication skills Excellent documentation skills for CRM/EHR systems Bilingual: English/Spanish preferred Proof of current CPR/BLS certification required or requirement to obtain one within 30 days of being hired Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law Schedule/ShiftM-F 8a-5p normally. Might require occasional evenings and weekends. LocationLos Angeles, CaliforniaThis role is hybrid, working remotely in the field and onsite at the Los Angeles facility as needed. We anticipate this role will be onsite up to 5 days per month. Salary Range$101,920.00 - $124,800.00
