Duties:Provide behavioral health counseling servicesConduct patient assessments and establish therapy treatment goalsAdminister individual and group therapyMaintain thorough and confidential records of observationsEvaluate progress and effectiveness of therapy interventionsProvide patient with information regarding resources and therapeutic activities in the communityWork directly with the patient, family, provider and community providers to ensure active and collaborate careProvide assessment, social services and counseling to patients, and their families in relation to social, psychological, financial and family situations.Provide individual, group and family psychotherapyDocument therapies and treatmentAssist with direct patient care as neededInitiate and conduct educational programs for staff as neededMarket and provide community education on therapeutic servicesDevelop and maintain good working relationships with outside community health and social agencies and reaches out to the community to develop new resources for meeting patient needsConduct training programs with outside organizations regarding health and wellnessFunction as lead therapist regarding trauma and personality disorders, providing clinical consultation to staffProvide clinical consultation to clinical staff about mental health issues for various patient populations, including geriatric and substance use disordersInterpret and communicate necessary information regarding patients and families including social history information to staff and providersCoordinate planning, make recommendations, and facilitate decision-making concerning individual patient care plans especially regarding the home situationPlan treatments and review provider casesPerform other duties as assigned
Schedule: Monday - Friday, 8AM-5PM
