Job Summary And Qualifications

We are searching for a LPN for a position on our patient-centered Behavioral Health team, responsible for assessing, evaluating, planning, implementing and coordinating a treatment plan and achieving the goals of the department. You serve as a key position of the interdependent team and hold responsibility for achieving positive patient and departmental outcomes.

What you will do in this role

You will manage efficient flow of patients including screening, preparation for exam, providing general nursing care, and arranging or testing and admissions You will instruct patients and families regarding medications and treatment instructions You will maintain and review patients’ records including posting tests and examination results You will administer medications in accordance with physician orders You will respond to and refer incoming telephone calls (may include phone triage when needed) You will collect data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. You will contribute to the enhancement of group effectiveness by exchanging information, collaborating with other team members, and providing constructive feedback. You will help develop team cohesiveness by offering assistance to team members and aligns individual work efforts to meet work unit and organizational goals for service excellence.

Minimum Qualifications

Must be a graduate from an accredited school of Licensed Practical Nursing Must be licensed as a Licensed Practical Nurse in the state which you practice and/or hold an active Compact/Multi-State LPN license A current American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) Health Care Provider is required
