Community Supports Coordinator-Southern CA

Position Summary

Build awareness about the Community Supports (CS) program and Medically Tailored Meals (MTM) from Mom’s Meals (MM) throughout California communities and the importance of nutrition and its impacts on Medi-Cal beneficiaries. Possess a passion for ensuring that beneficiaries are aware of and able to access available services that can improve their lives. Create an organized network of contacts and target organizations within internal systems. Engage and participate in the CA Department of Health Care Services (DHCS) county and health plan webinars. Attend and grow network of community events. The successful candidate will drive an increase in CS MTM referrals from a variety of organizations throughout CA. Build strong relationships with Community Based Organizations, Enhanced Care Management (ECM) Providers, Clinics, Hospital and Health systems, etc.

This position will be based in the southern CA area so candidate must reside in that area. It will also be eligible for a company car.

Salary range: $60,000-80,000

Position Responsibilities may include, but not limited to

Outreach to ECM providers, Federally Qualified Health Centers (FQHCs), hospitals, Long Term Care (LTC) facilities and Community Health Worker (CHW) organizations to engage and provide overview of MM servicesCreate and maintain target organization contact listEnsure all contacts and locations for all target organizations are up to date in internal systemOffer and schedule onsite (preferred) or virtual in-services with target organizationsBe highly visible within CS communities by engaging in community-based eventsAttend and participate in county round tables and county and state webinars to represent Mom’s MealsVisit Provider groups: Maternal Health practices, behavioral health, community transitions, local health clinics, FQHCs, WIC locations and others and provide overview of MM servicesStay up to date on referral workflows and Populations of Focus from CA DHCS and contracted health plansStay up to date with Community Supports integration with closed loop referral systems and Mom’s Meals presence within systemsStay up to date on DHCS and health plan Community Supports reporting requirements and work internally to ensure reporting meets requirements

Required Skills and Experience

2 years of community health workDemonstrated ability to successfully engage community health organizationsProven ability to work with cross functional teamsExcellent oral and written communication skills; experience giving presentationsExperience using Microsoft Office suite, including PowerPoint and ExcelWillingness to travel regionally to events, up to 50% travel
