Job Title: Primary Care Registered Nurse
Reports to: Health Education manager
Salary: Starting at $32.43/hr *salary commensurate with experience
Job Summary: Responsible for providing health education and care management for primary care health center patients through outreach, one-on-one appointments, care track consultations, and group classes. Assists medical providers with triage, monitoring, and assessment of clinic patients. 40 hrs/wk – no weekends or holidays!
Job Qualifications
- Licenses/Certifications:
o Associate degree in Nursing required, BSN preferred
o Active Utah RN license required
o Current Basic Life Support (CPR) certification required
o Proof of current immunizations will be requested upon hire
- Nursing experience in a health center setting preferred
- Experience with patient health education
- Bilingual: Spanish/English required
- Excellent interpersonal communication and customer-service skills.
- Ability to identify, prioritize, and complete tasks with limited supervision
- Reliable transportation. Occasional travel to other clinical sites within Utah County is possible.
- Basic computer skills and experience with EMR documentation.
Essential Duties
- Provide language-specific oral and written education to patients on the following topics:
o Diabetes management using ADA and DSMES criteria
o Prenatal education
o Cardiovascular health including blood pressure management
- Contact and schedule patients for transitional care management and other value-based care initiatives as needed
- Assist providers with Medicare Annual Wellness Visits
- Accept and provide warm hand-offs in the care track for health education
- Perform obstetrical Non-Stress Tests and consult with OB providers regarding results
- Collaborate with OB providers to provide triage for pregnant patients
- Track and follow-up on bilirubin results for newborns
- Coordinate with other community healthcare providers to provide preventative screening exams.
- Daily follow-up on clinical messages, lab work, and patient inquiries as needed.
- Attend assigned meetings
- Document all care and communication in the EMR (Athena)
- Maintain skills and knowledge through training, required meetings, and supervisor check-ins
- Follow MCHC policies, procedures, and standards. Display professional appearance and behavior.
