Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Case Manager to join our dedicated team. Since our inception in 1966, we have remained committed to our mission of providing accessible, affordable, and high-quality primary healthcare services to all, regardless of their financial circumstances.

As Case Manager, you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your experience in providing intensive case management services to high risk pregnant women and their newborns will further solidify our position as one of the largest providers of community-based primary care in the city.

Key Responsibilities

Coordinate Care: Work with high risk pregnant women and their newborns to assure a healthy delivery and postpartum care by providing intensive home visitation and case management services. Caseload Management: Maintains an assigned caseload. Assess: Completes individual assessments to determine the level of the participant’s health & service needs. Develop Care Plans: Develop care plans and resource identification as applicable. Referrals: Link participants to needed services; monitors service delivery and participant compliance. Record-Keeping: Document participant encounters and data (Includes electronic health record and cornerstone systems documentation).

Qualifications

MSW, MA, MPH, LPC, LCPC, or RN 2 years of experience in community health or maternal & child health OR 1-year experience in child, family or community services. Good organization and problem-solving skills. Typing and/or basic data entry skills are required. Proficiency in basic counseling skills, knowledge of community resources, and area service providers. Able to read, write and speak English. Good oral, and written communication skills required.
