As Intake Coordinator , you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your experience in Social and Human Services as well as your knowledge of community resources will further solidify our position as one of the largest providers of community-based primary care in the city.

Key Responsibilities

Intake: Conduct Intake into Home Visitation Programs using a universal tool to determine which home visit program that participants are eligible and has availability for the participant. Enroll: Conduct Enrollment into The Electronic Medical Records System(CPS12) Visit tracker and Corner Stone. Referrals: Support referrals to all home visiting programs to ensure they maximize a caseload capacity of 85% a) Monitor all program capacity levels and send referral to best-fit programs as needed to ensure caseloads do not drop below. b) Maintain a waiting list as needed when programs reach maximum capacity. Outreach: Know, engage, and collaborate with community partners and ancillary organizations related to enrolling eligible families in home visiting. a) Identify eligible families in the target community. b) Know the eligibility criteria for each HV program, and Know the capacity for each program c) Know the program models well enough to make an appropriate match between the family and HV program. d) Transition and support the entry into HV programs (warm handoff). Collaborate: Learn, engage and collaborate with key individuals, organizations, and ancillary agencies that support maternal, child and family health in the community we serve.

Qualifications

BS/BA in Human Services field. CPS12, Visit Tracker, and Corner stone Two or more years of experience in Social/Human Services field. Program experience and some quality improvement and quality assurance experience desirable. Demonstrated knowledge of community resources and social services providers. Excellent verbal and written communication skills. Computer operations and data entry skills.
