📋 Role Overview & Responsibilities

Position Summary: The Office of School Health (OSH) is a joint program of the NYC Public Schools (NYCPS) and the Department of Health and Mental Hygiene (DOHMH) responsible for the health of over 1 million school children enrolled in approximately 1,800 public and non-public schools in New York City; services to students include case management of chronic health problems, preventive health screening, urgent care, medication administration, preventive counseling, health education, referral for care and assurance of ongoing effective treatment. OSH has been the provider of public health services for over 100 years.

OSH manages the health concerns of students that have the potential to impact academic success. Some cases necessitate closer tracking of services, as well as family and school guidance and support in the implementation of those services. Clinical case management can include but is not limited to assessment, care coordination, care planning, in-school visits, and referral, for optimal health and academic outcomes. The Case Management unit ensures that issues/concerns encountered or raised by students and their families are addressed. The Community Engagement Coordinator will assist in the process of connecting families and students with support services through outreach efforts to parents and community partners. Performs related work.

Key Relationships: Works closely with the Regional Administrative Assistant, Medical Unit Team, and DOHMH/NYCPS agents.

Responsibilities

Develop and maintain ongoing relationships with families and community-based organizations (CBOs) to share information about supplemental services and identify community resources.Assist in the implementation and maintenance of processes that encourage referrals to programs and services and provide information to parents about NYCPS supplemental services. Develop and implement ongoing mechanisms to illicit input from CBO’s and families to meet identified needs and promote student success.Work with Medical Unit Leadership to develop specific and measurable program metrics for operational and performance management.Participate in ongoing meetings with the medical staff to plan for specific program implementation to ensure program sustainability.Prepare routine reports and presentations of program performance with data graphs/service trends; clearly communicate (spoken and written) results of analyses to program staff/leadership.Identify and implement strategies to correct or eliminate barriers to access and utilization of NYCPS supplemental services by analyzing data on program effectiveness and trends.Respond quickly and accurately to ad hoc data requests from Medical Unit Leadership.

Qualification Requirements

Minimum

A baccalaureate degree from an accredited college and two (2) years of experience in community work or community centered activities in an area related to the duties described above; orHigh school graduation or equivalent and six (6) years of experience in community work or community centered activities in an area related to the duties as described above; orEducation and/or experience which is equivalent to “1” or “2” above. However, all candidates must have at least one (1) year of experience as described in “1” above.

LMSW/LCMSW.Clinical case management experience, medical setting preferred.Excellent MS Office skills (Outlook, Word, Excel and PowerPoint).Strong organizational and communication skills.Excellent customer service skills to assist in resolving any issues. Strong verbal and written communication skills for both internal and external communications.Strong attention to detail.

Salary: $67,810 - $94,279

(Internal candidates who are selected for this position and who currently hold comparable or less senior positions within the DOE will not earn less than their current salary.)
