Job Description

PURPOSE OF POSITION

Under the supervision of a Social Work Supervisor Level V who provides social services to patients in the outpatient clinic environment.

Duties And Responsibilities

Assists patients and their families to obtain support services needed to maintain good health and function effectively in the community and documents accordingly. Support services constitute some of the following, entitlement assistance, SSA, SSI, Public Assistance, Medicaid/Medicare, Housing, Food Stamp Application, Transportation services, VNS, Home Care forms, etc.Completes comprehensive assessments on patients who are 65 years and older, HIV+ and prenatal patients.Confers and consults with professional personnel in implementing a multidisciplinary team approach to patient care.Provides direct social work services to high-risk and complex cases, e.g. domestic violence, child abuse/elder abuse.Provides supportive counseling and crisis intervention to patients/families/significant others as appropriate. Advocates for patients with various community agencies.Facilitates and coordinates education/support groups for patients/families/significant others as appropriate. Documents appropriate entries of contact intervention in patient's medical records as indicated by Social Work and Health Information Management Policy and Procedure.Completes and submits required report and/or statistics to the Department of Social Work.Participates in required social work meetings.Attends unit meetings, month Social Work Department meetings, in-service trainings and conferences. Attends scheduled supervisory conference with an appropriate agenda.Provides back up coverage for assigned worker(s).Performs other related duties as assigned by Social Work Supervisor.Promotes a positive and pleasant attitude conducive to the workplace, which enhances and furthers the Mission, Vision and Value Statement.Meets all compliance standards/requirements such as annual physical examination, assessment Annual in- service training, Fire Drill participation, etc.Participates in Performance Improvement Activities.Is responsible for enhancing his/her personal and professional development through attendance at outside and NYC H&H training sessions.Is responsible for wearing professional attire to the workplace as well as during off site work when conducting business on behalf of NYC H&H.Exhibits flexibility within the NYC H&H as it pertains to work assignment, workplace and site designation.

Minimum Qualifications

For Assignment to Level II

Valid license as an LMSW issued by the NYSED; or Valid license as an LCSW issued by the NYSED.

QUALIFICATIONS FOR THE JOB

1-2 years' experience or more working in an Ambulatory Care Setting with pediatric and adolescent patients

EDUCATIONAL LEVEL: Master's Degree in Social Work (LMSW) from a CSWE Accredited School/Program

Knowledgeable In

Knowledge of hospital systems and community resources Knowledge of federal, state, local and hospital standards, regulations, guidelines, policies and procedures

Knowledge of Medicare and Medicaid regulations

Knowledge of Substance use/abuse: behaviors,

interventions and resources

Knowledge of Child Abuse and Neglect: how to identify and report

Knowledge of Domestic Violence, Sexual Assault/Rape and Elder Abuse: intervention and resources
