This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 7:30am – 4:00pm EST. It may be necessary, given the business need, to work occasional overtime or weekends. Our office is located at 489 State St Bangor, ME.

We offer 4-6 weeks of on-the-job training. The hours of the training will be aligned with your schedule.

Primary Responsibilities

Proactively facilitates referral, transition, ordering of equipment (DME), and placement. Coordinates with key stakeholders and expedites final transfer with staff, patient, family, and facility.Maintains contact with appropriate facilities to advocate patient admissionsParticipates in family and interdisciplinary team meetings as appropriate to develop and execute the discharge planMaintains a directory on referrals, admissions and homecare / community agency resources and tracks discharge process utilized by the patient. Maintains current information on non-acute provider agencies, including SNF, sub-acute, acute rehab and chronic facilities, including programs, homecare and specialties available. Acts as a resource to staff, patients and families concerning this information.Updates the staff on new facilities and services and maintains a library of reference materialsAttend LOS meetings and follow ups on key tasks to ensure efficient facilitation of patient care as delegatedEstablishes work priorities with collaboration of assigned Care Management TeamAssumes responsibility for NL EMMC required continued education and owns professional growthPerforms other duties as assigned or required

Required Qualifications

High School Diploma / GED OR equivalent work experienceMust be 18 years of age or olderExperience in health care facilities, DME companies, or hospitals Flu vaccination are an essential job function of this role. Candidates located in states that mandate Flu vaccination doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state, and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Candidates must be able to perform all essential job functions with or without reasonable accommodation. Ability to work our normal business hours of 7:30am – 4:00pm EST.
