📋 Role Overview & Responsibilities
The Lead Care Manager (LCM) coordinates and implements organization-wide Enhanced Care Management. In this capacity, the LCM oversees and implements the provision of Enhanced Care Management (ECM) services for plan members under this LCM’s care.If you are passionate about working with underprivileged and marginalized populations, seniors, and disabled adults, helping them to overcome a loss of self-worth, enhance their quality of life, and preserve their dignity and self-esteem, we want to talk to you!The position will allow you to work with the best healthcare professionals in your field who have worked in our industry for many years by providing case management and advocating for their clients.
Qualifications· High school diploma or equivalent required; bachelor's degree in a related field preferred.· Experience working with vulnerable populations.· Microsoft Office product expertise required (Excel, Word, PowerPoint, Outlook)· Bilingual· Strong interpersonal and communication skills, with the ability to connect with diverse populations.· Empathy, cultural sensitivity, and a passion for improving community health and well-being.· Excellent organizational skills and attention to detail.· Ability to work independently and collaboratively within a team.· Valid California Driver's license.
