Job Title: Nurse Practitioner - CareSight (Longitudinal Care)

Location: Rochester, New York

Salary: $125,000 - $135,000

Sign-On Bonus: $5,000 Upfront with claw-back within the year

Position Overview: We are seeking a dedicated Nurse Practitioner to join our care team in New York. You will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. Key Responsibilities

Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.

Diagnosis and Treatment: Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.

Chronic Disease Management: Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.

Medication Management: Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.

Preventive Care: Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.

Patient Education: Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.

Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.

Documentation: Maintain accurate and up-to-date electronic health records (EHR) to track patient progress, treatment plans, and interventions.

Follow-Up: Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.

Quality Assurance: Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.

Qualifications

Current licensure as a Nurse Practitioner in the state of New York. Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner. Minimum of 3600 hours of clinical experience with 1 year of clinical experience in a primary care or longitudinal care setting. Strong clinical assessment and diagnostic skills. Excellent communication and patient education abilities. Proficiency in electronic health record (EHR) systems.

Preferred Qualifications

HEDIS/STAR measures experience Homebased visit experience Hierarchical Condition Category (HCC) documentation experience

General Physical Requirements

Ability to complete physical requirements of the job including navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment.

Schedule

Working Hours: Our standard business hours are from Monday to Friday, 8am – 5pm, with a one–hour lunch break. Weekly Visits: Our minimum workload is 25 visits per week.
