📋 Role Overview & Responsibilities
General Summary/Responsibilities
The medical social worker/team leader provides direct social work intervention, transition planning, and counseling to patients and their families. The medical social worker/team leader assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker/team leader assists patients, families and the public who request assistance in making appropriate linkages to community resources and provide consultation to those seeking input and guidance in an effort to resolve patient concerns. The medical social worker/team leader, under the direction of the manager, will coordinate and oversee the activities of the social work team, providing daily coordination of team workload and assignments as well as resource to team members regarding difficult clinical and operational issues. Administrative duties include assisting in selection of and orientation of new employees, monitoring team performance, participation in leadership with manager/director.

Minimum Level of Education Required: Master's degree

Licensure/Certification/Registration
Licensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist Board. LISW required.

Strong Communication, leadership, organization and problem-solving skills. Must possess expertise in social work practice and have strong clinical skills and knowledge of community resources. Must have flexibility, function independently, have the ability to lead and guide a work group
