Pay Range: $38 - $42/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities
Outreaching member to enrol them in CM program.Building care plans, medication review, assessments care plans. 20 calls/day.System-specific training.
Key Requirements and Technology Experience
Case Management that has remote experience.Home Health Care/Hospice, but not their only experience.Time Management (work fast-paced environment).Works independently.Work remotely in telephonic CM.Education Requirement: Associate degree.Software Skills Required: Microsoft Office Suites.Technical skills.
