Foster Care Home Specialist

The Foster Care Homes Specialist uses their abilities, under supervision, to support resource families in providing safe, secure care for children in out-of-home placement. They engage and assist prospective resource (Kin relative, foster and adoptive) families with support and resources for children. Completes written documentation and licensing requirements per DHHS guidelines for resource (kin, relative, foster, and adoptive) families. Completes written annual documentation and licensing requirements for resource families, such as annual training requirements.

Provides TIPS-MAPP and Deciding Together preparation for prospective resource families. Provides training regarding resource (kinship foster and adoptive) care, as needed. When a home moves to adoption, ensures the Adoption Training as required by contract is completed by the Resource Family. Assures that conferences and related activities are conducted within standards. Completes and distributes written documentation accurately and timely to include monthly reports and support plans

Facilitates conferences and/or assists staff in conducting conferences and related activities. Liaison with DHHS, the courts, community resources, and families. Provides services according to compliance guidelines. Participates in team meetings. Conducts required number of visits to the resource home per month. Assures the resource home has what is needed to provide services to the children in the home. Knowledgeable of resource families’ strengths and needs in order to assure appropriate matching of children to be placed in their home. When matching a child with a prospective home, the placement team and supervisor is consulted and in agreement with the match. OR Obtains Supervisor approval when matching a child with a prospective home.

Consults with the placement team regarding any decisions when a child needs to be moved from a home or go to emergency respite. OR Obtains Supervisor approval regarding any decisions related to a child being moved from a home or going to emergency respite. Completes and maintains the annual training requirements for FCH staff (20 hours). Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms. Is knowledgeable of and follows all safety procedures. Reports unusual incidents through appropriate Risk Management, clinical and safety channels. Ensures clients’ rights are protected.

A Bachelor’s degree in human services related field preferred. Must be 21 years of age Must pass a drug screen, Nebraska State Patrol Criminal Background Check, DHHS APS and CPS Central Registry Checks, and Nebraska Sex Offender Registry.
