📋 Role Overview & Responsibilities
POSITION SUMMARY
The Sepsis Nurse Navigator facilitates the development, implementation and ongoing adherence of Sepsis bundles, protocols, and goal driven therapies. Ensures the protocols and therapies become the standard of care within the facility. Collaborates with leaders and physicians throughout the organization to review sepsis processes, monitor compliance with core measures and send feedback on performance to increase compliance with sepsis bundles and identify opportunities for improvement for better patient outcomes. MINIMUM QUALIFICATIONS
Education: Bachelor’s Degree in Nursing (BSN): Preferred
Experience: Minimum 3-5 years of Acute care, Critical Care, and/or ED experience.
Knowledge and Skills: Effective communication skills. Experience working with electronic medical records (EMR). Fluent computer skills including Excel database, graphs, and PowerPoint. Solid grasp of hemodynamics, basic infection control principles.
License/Certifications: Valid CA RN license required. Current American Heart Association BLS card and ACLS required.
Equipment: As appropriate for clinical area
Physical Requirements: Must be able to perform the essential physical requirements of the job.
