Position: Case Manager; Care Manager

Salary: $46,500 a year plus full healthcare coverage to include dental, vision, paid holiday/vacation/sick time and mileage stipend. Opportunity to receive a total of 5,000 in bonus each year. Healthcare, dental, vision and short-term disability to take effect upon 60/90-day review after start of employment.

Date Needed/Time: Actively hiring. Open until filled. 5-day work weeks with a rotating weekend schedule. Rare but potential after-hour due to emergencies.

Location: In-person within Sarasota and Manatee Counties

About the Position

This is not your typical case manager position- you will be making a noticeable difference daily by working directly with formerly homeless individuals seeking housing stability and recovery from mental illnesses.You will do this by helping residents learn life skills related to living in a home environment, helping them identify, develop and achieve personal goals related to physical, mental and social needs. We are looking for someone who will champion those we serve, implement received training and can think outside the box. Role Responsibilities· Screening of potential residents.· Organize and complete duties related to move-ins, relocation, and discharge.· Regular visits to homes and work with residents on goals. This includes coordinating with other community resources.· Enforce program rules through accountability.· Maintain case plans and notes on all program participants in accordance with professional standards as assigned by Second Heart Homes.· Assist residents with maintaining homes through teaching life skills.· Occasional transportation of residents to appointments with possibly advocacy.· Purchase and deliver house supplies using SHH credit card.· Collect dues and issue receipts.· Create and deliver workshops at program homes.Qualifications· Proof of a valid driver license and proof of insurance upon start date. Maintain reliable transportation, not to include public transportation.· Proficient in the use of the Internet and computer programs (i.e., Excel, Word, databases, etc.).· A bachelor’s degree from an accredited school of higher education (experience could substitute degree requirements however minimum possession of a high school diploma is required). Experience will be verified.· Knowledge of the resources available in the community to serve our residents.· Preferred, at least two years of experience working with literally homeless individuals.· Preferred, experience in a social service setting and/or mental health field.· Flexible to respond to emergency situation after normal business hours, when needed.

Physical and other position requirements:The ideal candidate must be able to complete all the physical requirements of the job with or without a reasonable accommodation.· This position requires work while seated and/or standing at a desk or at multiple locations, including the use of a computer.· This position requires the ability to lift a minimum of 30 pounds.· This position sometimes requires work under stressful conditions and during irregular hours.· Some duties require periods of working outdoors with residents. Must have the ability to stand and walk for extended periods.
