About the Position:The Sleep Care Manager will provide remote patient monitoring services to patients and help to manage patients’ treatment adherence in a remote, virtual-care setting. This individual will leverage Somnea Health’s remote patient monitoring technology while working collaboratively with a dynamic care team. This role is remote.
What You Will Be Doing:Provide ongoing sleep monitoring virtually to manage and support patients’ sleep needsApply clinical expertise and knowledge of CPAP adherence strategies while leveraging technology to help patients with sleep apnea remotely monitor their conditionUse behavioral and motivational strategies to coach patients on best practices for CPAP use and sleep hygiene to help them sleep successfully and live fullyCoordinate sleep apnea disease management for patients with their broader care teamRecord patient information concisely, accurately, and completely
What We Are Looking For:Experience in remote patient monitoring and telehealth, demonstrating proficiency in managing patients' treatment adherence in a virtual setting.A proactive approach to patient care, with the ability to educate and coach patients on CPAP usage, sleep hygiene, problem-solving strategies, and other practices essential for successful sleep therapy.Exceptional communication skills, capable of building trust and rapport with patients, while also collaborating effectively with a multidisciplinary team to coordinate comprehensive care plans.The ability to meticulously record patient information, ensuring accuracy and completeness for effective monitoring and management.A passion for healthcare and a personal drive to contribute meaningfully to the well-being of individuals living with sleep disorders.
Requirements:2 years of college or 3-5 years of experience in sleep medicineCurrent polysomnographic technologist license or other licenses including polysomnography in the scope of practice (RPSGT)Qualified to obtain or possession of the CCSH (Certification in Clinical Sleep Health) preferredStrong communication skills and ability to navigate team dynamicsProficient in using digital tools and platforms for healthcareProven experience being a quality, patient-focused caregiver, with content expertise in sleep disorder treatment and management
