Description

POSITION SUMMARY

Provides consultation, evaluation, and physical therapy services to patients across the age continuum. Establishes a rehabilitative plan of care with realistic goals following a functional evaluation for all patients. Provides education to patients, family members, students, staff, and to the community at large. Coordinates services with other medical disciplines, making recommendations as appropriate to facilitate a complete rehabilitative process. Participates in Performance Improvement and Risk Management activities.

Education, Training, Experience, Certification And Licensure

Graduate of a physical therapy program approved by the American Physical Therapy Association. Currently licensed or eligible for licensure in the State of New York.

Maintains current BLS/CPR certification.Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise as applicable, including but not limited to:Hip, Knee replacement ProgramStrokeWound CareHeartburn Center

Special Equipment, Skills Or Other Requirements

Strong oral and written communication skills. Strong interpersonal skills. Strong time management skills. BLS for Health Care Providers preferred.

Work Environment And Hazards

Clinical setting. Exposure Class I – Routine or potential exposure to blood, body fluids, excretions or secretions.

Physical Demands

Medium work: Continual and repetitive motions, including but not limited to, medium lifting, bending, stooping, twisting, pushing, pulling, climbing and prolonged walking and standing.

All services, employees, medical staff, patients, visitors, vendors, various regulatory and professional agencies.

Service Coordinator or Manager

Supervises

Physical Therapy Assistant and Rehabilitation Aide. Student Physical Therapist and/or student Physical Therapy Assistant, after one year clinical experience.

Pay Rate: $47.95/hour

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location
