Position: Care Advocate Representative
Location: Kapolei, HI (hybrid)
Pay: $16/hr to $20/hr (depending on experience)

Education: High school diploma or GED
Type: Full-time; Contract
Schedule: Monday - Friday, 8 a.m. to 5 p.m.

Job Description
Provide support to the provider appointment adherenceAssist PCP office and focus on quality measures Support office and practice transformation in improving workflows, tools and processesWorks internally with HMSA Integrated Health Management Services program teams and educates PCPs on programs offered to their patientsBe able to explain and inform PO and PCPs and their office staff of the programs and managed care offered by HMSAWorks on internal HMSA projects related to Payment Transformation, HEDIS and Medicare STARSEngage in member outreachWork in various platforms and systems to ensure end to end gap closure
Position Requirements
Proficient with PC and Microsoft OfficeAbility to multi-task, learn new tasks quickly and retain informationMust be self-directed and work well with deadlinesDemonstrate initiative and handle tasks efficiently Highly organized, good time management and effective written/verbal communication skills Be able to work in a team and independently, displaying critical thinking, problem solving and resourcefulnessValid driver’s license and an access to an automobile Able to pull/push up to 25 pounds and able to lift up to 5 poundsMust provide proof of being fully vaccinated against COVID-19 (unless a reasonable accommodation is approved)
