Hospice Licensed Practical Nurse
We are seeking for a compassionate Hospice Licensed Practical Nurse to provide care and support to terminally ill patients. Job Responsibilities
The Hospice LPN will provide compassionate nursing care and education to our patients and families under the supervision of the assigned RN Case Managers, and at times support the clinical team from the office.Complete appropriate documentation in timely mannerAssist in auditing/compliance functionsAssist with monitoring and ordering patient care suppliesInform the physician of changes in the patient's condition and needs, after consulting with a Registered NurseCommunicate with medical professionals, hospice team and related staff
Qualifications
Graduate of a Practical Nursing ProgramValid LPN License in the stateMinimum one year of experiencePrevious experience in hospiceWillingness to serve on-callMust have reliable transportation & valid driver's license
