They provide a wide range of supports, based on the specific needs of the patient and the patient's family. They advocate, problem solve, listen, and connect people to resources.They also provide resources such as counseling and referral services.They help families cope with the stress of dealing with a terminal illness and coordinate care for those who may be facing financial or other hardships due to the illness.
