Position Overview

The role is to act as the work from home clinical liaison that represents the agency via telephone receiving and processing referrals for patient care in the regional home health referral management team. Referral source interactions include, but are not limited to working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. Primary clinical responsibilities include receiving/processing referrals and ensuring that admissions occur in a timely and consistent manner. This position enters referral information into the Electronic Medical Records (EMR) and creates initial orders to facilitate the start of care. As the Clinical Referral Specialist, this person frequently interacts with referral sources, other members of the referral center staff, home health, Advanced Illness Management (AIM) and hospice staff across offices and all others necessary to facilitate the start of patient care. Consistently utilizes and promotes use of excellent customer services skills. This position reports directly to the Home Health Administrator

Bachelor's: Nursing or health related field preferred or equivalent education/experienceCERTIFICATION & LICENSURERN-Registered Nurse of California (Can be acquired after hire)RN-Registered Nurse in State of ResidenceBLS-Basic Life Support Healthcare Provider

Preferred Experience As Typically Acquired In

2 years experience RN: in an acute or similar setting2 years experience RN: In a home health position2 years experience in direct home health patient care2 years experience in coordinating/processing home care referrals

Must have the clinical knowledge and critical thinking skills to effectively plan and provide coordination of patient care consistent with standards and regulationsMust have exceptional interpersonal and customer services skillsMust be able to effectively solve unique problems as they arise or identify when to consult the supervisorMust have knowledge of current Hospice/Home Health admission criteria; Medicare, Medi-Cal, Commercial, State and Federal regulations Familiarity with ICD-10 coding preferred but not essentialMust be able to demonstrate problem-solving abilities as well as telephone, interpersonal, verbal and written communication skills in EnglishCommitment to teamwork and customer service, both internally and externally, is requiredMust have proven /proven data entry and general computer skillsMust be able to demonstrate proficiency (after training and introductory period) in efficient use of electronic medical record systemsMust be able to flex with the increased workflow when census is higher and recognize urgency of each taskMust be flexible with schedule including, but not limited to, the ability to participate in department rotation for weekend coverage if neededMust be able to drive to designated SCAH location(s) for meetings, training, and needs as designated by business needs

Pay Range is $33.06 to $43.97 / hour

The salary range for this role may vary above or below the posted range as determined by location.
