📋 Role Overview & Responsibilities
Location: Hiring for 4 Boroughs (Manhattan, Bronx, Brooklyn, Queens)Schedule: 3 days doing home assessments within your borough & 2 days remote, telehealthPay: $48-$52/hr.
Job Summary:Evaluate health status questions and make determinations based on UAS criteriaUnderstand state and agency laws and regulations governing nursing practices
Responsibilities:-Conduct initial in-home evaluations of patients, developing individualized long-term care plans-Review relevant medical documentation and enter evaluation data into electronic forms-Document concerns and issues that arise during evaluations-Discuss healthcare options and service vendors with consumers-Facilitate communication between Call Center and eligible consumers for enrollment-Collaborate with interdisciplinary care team members to improve care coordination-Collect quality review data and maintain case notes-Identify opportunities for health promotion and illness prevention-Stay informed about community resources, payor requirements, and network services-Meet established performance standards and criteria-Perform additional duties as assigned by Regional Nurse Manager or project management
Required Qualifications: All nurse assessors MUST meet the following requirements•Two years of experience in a HOME setting within the last ten years•Conducting assessments that are similar to the UAS such as OASIS•Providing primary care to homebound persons•Care management for persons receiving home and community-based services including Hospice and Assisted Living•Home care experience as a paraprofessional (e.g., Home Health Aide) or Licensed Practical Nurse caring for a minimum of 2 patients per week
