A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff in Manhattan as a Hospice Referral Coordinator.
Responsibilities
The Hospice Referral Coordinator (RN) will be responsible for:Evaluating and establishing a plan of care for patients referred for home health servicesFacilitating admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family membersDeveloping and maintaining positive relationships with Physicians, Social Workers, Case Managers and Discharge PlannersPerforming other duties, as needed
Qualifications:2+ years of experience in Home Care NursingGraduate of an accredited School of NursingNYS Registered Nurse (RN) licenseProficiency in Physical Assessment and Interviewing skillsSolid assessment, clinical, and documentationGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:Bachelor's Degree in Nursing
