Nurse Assessor (RN)

A medical services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their growing team as their new Nurse Assessor. In this role, the Nurse Assessor (RN) will be responsible for performing initial in-home pre-evaluation of patients, including evaluation of health status, strengths, care needs, and preferences and guide the development of individualized long term care service plans.Responsibilities:

The Nurse Assessors (RN) will:Review consumer medical documentation and or health referral forms, as relevant to the caseEnter evaluation data into electronic evaluation form and transmit, as required Document any concerns, conflicting information, other issues that surface during the evaluation process Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan Discuss with consumer on health care options, supports needed, service vendor options and waiver optionsInitiate communication between the Call Center and eligible consumers for enrollment into a managed care health planCollect quality review data and any required documentation to support outcome measurements and record case notes into CRM Identify opportunities for health promotion and illness preventionPerform other duties, as neededQualifications:2+ years of Nursing experience with Home Care background; or 1+ year of Nursing experience with UAS in Home CareGraduate of an accredited Nursing programNYS Registered Nurse (RN) licenseDesired Skills:Bachelor's Degree in NursingBilingual (English and Spanish, Russian, Haitian-Creole, Cantonese or Mandarin)
