If that sounds like you, apply now and become part of our team at Giving Home!We have an immediate need for a full-time Home Visit Specialist to join our Care Team.Our Face Visit Specialists work with our clients to provide information about the program that we work with, providing a personalized experience for each individual. They work closely with a care team to ensure that the client's needs are met in relation to their medical conditions while collaborating with providers offices throughout the community focusing on each client's plan of care. Additionally, they plan periodic visits to client's homes to update files for compliance purposes.
