Home Health Aide (CNA)- Hospice Float
Pay: Up to $31/hour depending on experience + $1,500 attendance bonus!
Location: This is a float position. Must be willing to work in the service area of Eastern Minnesota, primarily Stillwater, MN.
Schedule: Full-Time (1.0 FTE), 8am - 4:30pm M-F, on-call every 7th weekend.
Position Summary: Assists professional personnel to provide patient care according to hospice standards. Completes other supportive tasks necessary to the functioning of the department under the supervision of a Case Manager. This CNA will be assigned a client and then this staff will see this patient throughout the whole case until they pass away. The case manager will dictate the schedule and care plan and assign the CNA. The CNA is often working independently, without the nurse.
Essential Duties And Responsibilities
Assist with patient ADLs/vitals and complete accurate documentation.Assist with transferring patients safelyPrepare meals and assist with feeding patientsPerform or assist the client in performing activities related to mobility-passive range of motion, turning and repositioning.Assist the patient in taking self-administered oral medications.Perform light housekeeping duties to maintain the patient’s level of function: prepare light meals, change bed linens, clean and straighten the patient’s immediate personal environment.Report significant observations and changes in the patient’s condition to the CM within a timely manner.Provide Oxygen per patient's request.Maintain department order and equipment management as needed.Perform other tasks as assigned.

Requirements
High school diploma/GED18 years of age or older and have at least 1 year of driving experience.Prior nursing assistance experience preferred.CNA certificate and current status on the MN CNA RegistryCurrent BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)Ability to communicate in English in verbal and written form.Able to complete charting in electronic medical records systems.Ability to pass an MN DHS Background Study.
