📋 Role Overview & Responsibilities
Looking for Nurse Practitioners to complete Health Assessment!

Assessment Schedule:Availability: Assessments can be completed any day of the year (365 days) between 8:00 AM and 8:00 PM.Target: We aim for our providers to complete 5-6 assessments daily.Minimum: We require a minimum of 3 assessments to be completed per workday.
What You'll Do:Conduct thorough, hour-long assessments, focusing on current health, medications, social environment, and potential risks.Gain a deeper understanding of a member's well-being compared to traditional office visits.Contribute to improved quality of care and potentially close multiple care gaps in a single visit.Collaborate with physicians, case managers, and other healthcare professionals to ensure continuity of care.Provide patient education and follow-up support.
Qualifications:Master's degree in Nursing or equivalent experience and NP licensure.Current RN and NP license in the state of practice with prescriptive authority.Must be licensed in one of the following specialties:Acute CareAdult HealthFamily HealthOncologyBoard certification by AANP, ANCC, or AACCN in a relevant specialty.Current BLS, ACLS, or CPR certification.Minimum 1 year of experience as a Nurse Practitioner (New grads welcome!).Strong computer skills and experience with electronic health records.Excellent communication skills to connect with patients and colleagues.Ability to adapt and thrive in a dynamic environment.Valid driver's license, reliable car, and adequate car insurance.
